Job Summary :
The LVN provides direct nursing care to medically fragile residents in a CLHF setting. Under the supervision of an RN or physician, the LVN ensures high-quality, compassionate care tailored to each patients clinical needs.
Key Responsibilities :
- Administer prescribed medications and treatments accurately.
- Provide skilled care for residents with tracheostomy, ventilator dependence, G-tubes, and wound care needs.
- Monitor and record vital signs, intake / output, and patient status changes.
- Communicate with an interdisciplinary team regarding resident conditions and care plans.
- Document all care provided in accordance with state regulations and facility policies.
- Respond promptly to patient needs and emergencies.
- Assist with ADLs (activities of daily living) when necessary in coordination with CNAs.
- Support infection control and safety protocols.
Requirements :
Current LVN license in the state of California.Valid BLS / CPR certification.Previous experience in a subacute, CLHF, or skilled nursing environment preferred.Familiarity with ventilator and trach care is highly desirable.Strong documentation and communication skills.Ability to work independently and collaboratively in a small team setting.Physical ability to perform nursing duties, including lifting, bending, and long periods of standing.Shift & Pay :
Shifts : 12-hour shifts, the rate is based on competitive compensation, depending on experience.
