Contract Duration : 13 weeks

Schedule : Monday – Friday, 8 : 00am – 4 : 30pm (8-hour day shifts)

Hours per Week : 40

Weekend Coverage : As needed

Estimated Weekly Salary Range : $2,370 - $2,430 per week.

Shifts per week : 5 (Monday – Friday).

Registered Nurse Qualifications

Active RN license in New York State

Minimum 1 year of clinical nursing experience in a hospital (RN jobs)

At least 9 months of home health care experience preferred

Certified Home Health Care experience required

OASIS assessment experience required

Telemetry experience required

BLS certification (any provider accepted)

EPIC or Netsmart EMR experience preferred

Current NYS driver’s license

Strong interviewing, clinical assessment, and communication skills

Ability to exercise initiative, independent judgment, and work independently with minimal supervision

Critical thinking skills and ability to follow standards / procedures

Case Management RN Responsibilities

Develop and revise treatment and discharge plans with patients, families, physicians, and care teams

Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, and discharges within agency timeframes

Process medical orders and communicate patient needs and changes in condition to physicians

Provide specialized nursing services and initiate preventive and rehabilitative strategies

Educate patients, families, and team members on disease processes, self-care, and preventive strategies

Supervise LPNs and ensure compliance with care plans

Coordinate, evaluate, and supervise the quality of patient care services

Maintain accurate clinical records for compliance and seamless service delivery

Attend patient care conferences and participate in quality improvement initiatives

Promote a respectful, diverse, and professional work environment

Demonstrate flexibility and willingness to visit patients based on need
