Job Overview :
Interim House West is a residential substance abuse treatment program in Philadelphia for women and their children. The Family Services Coordinator supports program participants by completing intakes, developing service plans, coordinating case management, and communicating with DHS as well as other child welfare agencies.
Responsibilities :
- Coordinate all activities between Interim House West and child welfare agencies, including the Department of Human Services.
- Complete an initial intake with each mother and her children at the time of admission to the program.
- Coordinate family reunification plans with other members of the treatment team as well as child welfare agencies.
- Coordinate family case management activities.
- Develop and submit initial and follow-up service plans to DHS.
Experience :
Knowledge of community-based resources for children and families, who require specialized support.2 years experience in providing case management services to children and / or families.Requirements :
Bachelor’s degree in social work, early childhood education, or human services required.2 years of experience providing care management services to children and / or families required.Experience working with the Department of Human Services, child welfare agencies, etc.Experience working with children with delays in cognitive, communicative, or perceptual motor skills development.Knowledge of community-based resources for children and families who require specialized support.PHMC is and EOE and an E- Verify Employer
