Overview :
Under the direction of the primary care operations team, the Primary Care Patient Certified MA works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Patient Care Certified MA will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for all patients. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.
Job Description :
Additional monthly duties as assigned by clinic manger (e.g., injection refrigerator temperature logs, checking sample log daily, ensuring oxygen levels / tanks resupplied, expired lab supplies checked, order medical supplies, etc.).
Performs other work duties as assigned.

Minimum Qualifications
Education :
Certification :
Additional minimum requirement(s) only for Hospital Outpatient Departments
Certification(s) : BLS (Basic Life Support)
Preferred Qualifications :
Experience : One year of applicable patient care experience in an outpatient or ambulatory care setting.
Education : Graduate from an accredited Medical Assistant program
Preferred requirement(s) for non-HOD Clinics :
Certification(s) : BLS (Basic Life Support)
Knowledge, Skills, and Abilities :
= 90% for the last 6 months including :
AWV questionnaire and documentation.
Thorough Medication Reconciliation
Closing all open care gaps as indicated Encounter Guide (Fall Risk, Depression, Breast Cancer Screening, Colorectal Cancer Screening, and Diabetes Eye Exam)
Blood Pressure Recheck for patients with elevated initial blood pressure reading
Completion of Patient Experience Conversation
