Registered Nurse - Adult Foster Care Program

F ull-time, 35 hours / week

A Hybrid schedule with three days / week may be possible after the initial training.

Seeking team player to work as a full-time RN for our Adult Foster Care (AFC) Program. No nights, weekends or holidays. Responsible for nursing assessment, screening and evaluation of potential participants and care providers living in the community. Participates in team decisions matching eligible participants and families. Develops, implements, and monitors participants' nursing care plans. Provides healthcare training and education to program participants and foster families. Reviews clients' plans of care in conjunction with the primary care physician semi-annually for accuracy and updates. No hands-on care or heavy lifting required.

Minimum Qualifications : Valid MA RN license, BS in Nursing or Human Services preferred. Minimum one year experience, preferably in community-based setting. Must have valid driver's license and reliable transportation. Spanish fluency helpful but not required. Must be able to climb multiple flights of stairs.

Travel reimbursement is $0.70 per mile

Public Service Loan Forgiveness (PSLF)
