Job Description :

The Care Manager RN plays a pivotal role in providing comprehensive patient-centric care management services for at-risk patients within an acute care environment. This includes assessment and planning, coordination of care, resource utilization management and review, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.

The ultimate goal of the Inpatient Care Management RN is to ensure the judicious use of healthcare resources throughout the continuum, guaranteeing that the care provided is appropriate, timely, and setting-specific.

Care managers collaborate with multidisciplinary teams to develop, implement, and evaluate individualized plans of care, aiming to promote optimal health outcomes. They also facilitate communication among healthcare providers, payers, and community resources to support seamless transitions across the care continuum.

Key Responsibilities :
