Description :

The position requires experience in care coordination and discharge planning.

Candidates must have acute care hospital experience.

This role involves assessing patients and developing treatment plans.

Knowledge of drug / alcohol abuse treatment options is essential.

Experience with patients experiencing homelessness and mental health crises is also needed.

Ability to organize and participate in family meetings is required.

The candidate must be competent in arranging durable medical equipment, skilled nursing facilities, and ambulance transport.

Computerized documentation experience is necessary.

Proficiency in Microsoft applications is expected.

Candidates must be willing to float between various facilities.

All travelers are expected to complete required training modules.

Participation in the development of treatment plans is part of the job.

Providers must commit to working three of five designated holidays.

A minimum of two supervisory references covering one year of relevant experience is needed.

Experience with HMOs and IPAs is beneficial.

The candidate should be capable of utilizing InterQual criteria and managing plans of care.
