Position Summary :

The Family Nurse Practitioner (FNP) will play a vital role in delivering primary care services in an outpatient setting. This position is ideal for a clinician passionate about addressing health disparities and making a meaningful difference in the lives of patients who may face barriers to care. The FNP will collaborate with a multidisciplinary team to promote wellness, manage chronic conditions, and provide preventative and acute care services.

Key Responsibilities :

• Provide comprehensive primary care services for patients of all ages, including health assessments, diagnosis, treatment, and follow-up.
• Manage acute and chronic conditions, emphasizing patient education and self-management strategies.
• Order and interpret diagnostic tests, prescribe medications, and develop individualized care plans.
• Collaborate with physicians, nurses, behavioral health specialists, social workers, and other team members to ensure coordinated and holistic care.
• Promote preventative health measures, including screenings, immunizations, and lifestyle counseling.
• Document patient encounters accurately and timely in the electronic health record (EHR).
• Serve as an advocate for patients, connecting them with community resources and support services as needed.
• Participate in quality improvement initiatives and contribute to advancing health equity within the clinic and community.
