• Location Restrictions :
• Candidates with an Arizona (AZ) home address on their driver's license are not eligible ; a copy of the license must be included in the application.
• All requested time off (RTO) must be listed, cannot exceed 7 days, and is not approved for the first week or holidays.
• List all facilities, states, and shifts the traveler is open to working.
• All departments of interest must be checked on the cover page.
• Experience & Certification Requirements :
• Minimum of 24 months of recent Progressive Care Unit (PCU) experience required .
• Certifications (ACLS, BLS) must be American Heart Association (AHA) or American Red Cross, active upon submission, and included.
• No license violations (hits) accepted.
• Active Arizona RN license required at time of submission (verification must be
