Pay Range : $40 - $45 / hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities :

• Performs care management duties to assess and coordinate all aspects of medical and supporting services across the continuum of care for complex / high acuity populations with primary medical / physical health needs to promote quality, cost effective care.
• Develops a personalized care plan / service plan for long-term care members, addresses issues, and educates members and their families / caregivers on services and benefit options available to receive appropriate high-quality care.
• This position is for the McAllen / Edinburg area to complete assessments in member’s homes as well as completing documentation as per State and Company guidelines to ensure compliance.
• M-F visits 3 days out of the week, with OT possible with manager approval, documentation days 2 days out of week, traveling to members homes to complete assessments / documentation.
• Face to Face assessments completed 3 days out of the week with 2 documentation days, completing tasks, submitting all forms / assessments within turnaround times, completing outreaches
