JOB SUMMARY
Serving as a key member of the Front Door Diversion Program team, the Program Coordinator Lead will be responsible for the following :
Primary responsibility is to oversee, lead, and provide direct services to referrals received from PAS-R and community partners. Additionally, outreach and market to alternative sites for referrals to the Front Door Diversion Program. The program coordinator is responsible to facilitate continued engagement of consumers leaving EDs or Inpatient units without agreeing to alternative care options.
The program coordinator should have the capacity to conduct telephone or face-to-face outreach and assessments of referred consumers. Program coordinator will provide continuing care management for consumers accepting alternative care options until a hard hand-off is accomplished with the next-level care provider.

Salary : $58,000

Occupational Risk for Exposure to TB
This position involves direct patient contact, posing a risk of exposure to Tuberculosis (TB). Employment is contingent upon annual TB testing, except for :
Staff who decline TB testing or flu vaccinations due to medical or religious reasons will be required to wear a mask as appropriate.
