JOB SUMMARY

Serving as a key member of the Front Door Diversion Program team, the Program Coordinator Lead will be responsible for the following :

Primary responsibility is to oversee, lead, and provide direct services to referrals received from PAS-R and community partners. Additionally, outreach and market to alternative sites for referrals to the Front Door Diversion Program. The program coordinator is responsible to facilitate continued engagement of consumers leaving EDs or Inpatient units without agreeing to alternative care options.

The program coordinator should have the capacity to conduct telephone or face-to-face outreach and assessments of referred consumers. Program coordinator will provide continuing care management for consumers accepting alternative care options until a hard hand-off is accomplished with the next-level care provider.

JOB DUTIES

  • Provide community outreach and engagement via marketing and scheduled meetings with referring entities.
  • Submit accurate and on-time program reports to internal and external administrators. This includes Weekly Workbooks, Housing Authorization Report, Supplemental Invoice and all other reports as assigned.
  • Conduct daily / weekly meetings with staff to discuss program updates, referrals, and diversion outcomes.
  • Attend and participate with monthly DMH Quality Assurance visits, conducted to ensure continuity of care and services to all diverted consumers.
  • Provide ongoing supervision and trainings to staff.
  • Ensure staff are appropriately documenting services in the electronic medical record (EMR) system.
  • Assist staff with claiming, assessing and / or assigning referrals.
  • Refer diverted clients to appropriate mental health program(s) for ongoing case management services.
  • Participate in leadership meetings to provide program updates, barriers, and progress as it relates to FDDP.
  • Attend staff development training and seminars as approved by supervisor.
  • Establish relationships with key individuals, hospitals, and other referral entities within the community, especially as a resource for program participants.
  • Conduct and / or participate in speaking engagements to promote the program, as directed.
  • Secure participant agreements for referral sources.
  • Provide community-based case management to clients refusing treatment at the time of discharge from an In-patient setting.
  • Assist with completing general intake assessments for walk-in clients.
  • Attend Front Door Diversion meetings with the State of Illinois.
  • Other related duties as assigned by manager.

QUALIFICATIONS

  • At least 2 years of related clinical and supervisory experience. Master’s degree (preferred).
  • LSW, LPC preferred.
  • Knowledge of community / hospital associations / affiliations.
  • Excellent communication skills (oral and written communications).
  • Well organized, detailed- oriented and able to function in a fast-paced work environment.
  • Knowledge of computers and applicable software.
  • Valid Driver’s license.
  • SMART GOALS

  • Establish or formalize at least five new community partnerships with permanent housing providers, social service agencies, or substance recovery partners to enhance collaborative care options, documented through MOUs or partnership agreements.
  • By the end of each program year, meet or exceed contractual deliverables as evident by at least 44 Diversions, 95% 24-hr engagement, and “in-person” completed referrals.
  • Coordinate and deliver at least two training sessions for program staff on topics such as screening and assessments, cultural competence, or crisis intervention to enhance the overall quality of referral screenings.

  • Has a working vehicle
  • Work is in office and community- based
  • Moderate physical activity. Will require physical effort including frequent walking and occasional lifting up to 25-50 pounds as well as some extended periods of standing

    Occupational Risk for Exposure to TB

    This position involves direct patient contact, posing a risk of exposure to Tuberculosis (TB). Employment is contingent upon annual TB testing, except for :

  • Documented medical contraindications
  • Documented religious objections
  • Staff who decline TB testing or flu vaccinations due to medical or religious reasons will be required to wear a mask as appropriate.
