Your Role and Responsibilities

• Conduct thorough patient evaluations, determine necessary diagnostic tests, and create personalized care plans
• Diagnose and manage both acute and chronic conditions either in collaboration with physicians or independently as your credentials allow
• Engage patients and their families in discussions regarding prevention, wellness initiatives, and effective disease management strategies
• Prescribe medications and therapeutic measures as permitted by your scope of practice
• Participate actively in chronic disease management and patient follow-up
• Coordinate with multidisciplinary teams to facilitate comprehensive and well-rounded patient care
• Maintain detailed and precise documentation in electronic medical records (EMR)

Your Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Credentialing Requirements

• Valid Nurse Practitioner license in the state of California
• Nationally recognized certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or equivalent
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or willingness to qualify within 90 days)
