Description :

We are seeking an experienced RN Case Manager to join our team.

The facility has a high average census and patient case load.

The RN will typically handle 14-16 patients, depending on the unit assigned.

Candidates must have a minimum of 3-5 years of experience in Case Management.

First-time travelers will not be considered for this position.

The patient population may include adults from various medical / surgical backgrounds, cardiac, orthopedic, and potentially pediatrics.

Required skills include assessment, care coordination, and discharge planning.

Experience with the EPIC electronic medical record system is required.

The role involves complex placement and working with the homeless population.

The department is supportive, featuring a collaborative team environment.

Weekend rotation may be required, with scheduling dependent on the requisition.

Travelers will work all holidays unless approved otherwise.

The culture within the department is teamwork-oriented, with strong communication from management.

Educators provide support throughout any changes within the department.

Navy blue scrubs are required for this position.
