Nurse Practitioner NP

RiteMed Group, a provider-focused organization specializing in Nurse Practitioner and MD staffing in long-term care, is seeking a compassionate and skilled Nurse Practitioner for Optima Care at Hamilton Park, a skilled nursing facility. This is a full-time position with flexible salary or per-note compensation, offering the chance to practice in a supportive, team-oriented environment.

Key Responsibilities

Deliver on-site primary care to residents in both long-term and subacute units.

Perform comprehensive assessments, manage chronic illnesses, and respond to acute medical needs.

Collaborate with nursing staff, therapy, and interdisciplinary teams to optimize care.

Maintain thorough documentation in EMR systems.

Help reduce unnecessary hospital transfers while improving resident outcomes.

Qualifications

Active Nurse Practitioner license in New Jersey.

Minimum of 6 months NP experience (ICU or hospitalist background a plus).

Strong clinical judgment, communication, and collaboration skills.

Ability to care for complex patient populations in a skilled nursing environment.
