    • Weekend Requirements - CM Two weekends every six weeks

    Requirements :

  • Experience :

    • Minimum 2 years’ acute hospital Case Manager experience (Care Coordination & Discharge Planning).

  • Patient ratio experience : 1 : 20.
  • Prior travel assignment experience required.
  • Charting System : Epic (required).
  • Certifications :

    • BLS (required).

  • Additional case management–related certifications preferred .
  • Preferred Experience :

    • Teaching hospital, community hospital, LTAC.

  • Trauma Level I and / or Level II experience.

    • Skills & Work Settings :

  • Required :

    • Case Management / Utilization Review in acute hospital setting.

  • Preferred :

    • Ambulatory care

  • Behavioral Health
