As the Harm Reduction Case Manager , you will play a pivotal role in individuals' lives, making a significant impact through :

• Review assessments and discharge plans provided by medical staff, CASAC, or other facility staff
• Utilize assessments and discharge planning provided by correctional facility partner to develop a plan of care
• Provide Opioid Overdose Prevention training in accordance with agency guidelines
• Provide Enrollment into the agencies Syringe Program
• Provide HIV and HCV Screening as necessary

Harm Reduction Case Manager Minimum Salary : $42,000

Harm Reduction Case Manager Maximum Salary : $42,000

Harm Reduction Case Manager Schedule : Mon – Fri 8 : 30am – 4 : 30pm
