We are currently seeking a caring and experienced Hospice RN Case Manager to join our team to walk alongside as a supportive presence for patients and families beginning during their hospice journey. If you're an experienced RN with a calling to serve and a desire to truly connect with patients and families, we'd love to welcome you to our team. Responsibilities :

• Care for patients where they call home.
• Use your clinical skills and your emotional support skills to support patients and families.
• Create care plans that treat the whole person—body, mind, and spirit.
• Work closely with providers, referral sources, and our amazing interdisciplinary team (IDT).
• Be a steady, calming presence during a difficult time—and help others feel just a little more at peace.
• Educate families, empower caregivers, and be the go-to person when people have questions. Requirements :
• A current RN license in Michigan
• Hospice, home care, acute care, or case management experience
• Confidence working independently—but also a team-first attitude
• On call needs based on YOUR schedule (nights, weekends, holidays)
