The Home Health Manager provides mid-level operational and clinical oversight of the Home Health program. This role supports the Director of Nursing by supervising home health nursing and aide staff, ensuring regulatory compliance, reviewing OASIS documentation, coordinating interdisciplinary care, and maintaining efficient daily operations.
The Home Health Manager collaborates closely with the Therapy Clinical Manager, Intake Coordinator, QAPI, scheduling, and administrative leadership to ensure high-quality, timely, and compliant patient care . The role serves as a liaison between the agency, physicians, hospitals, SNFs, and other community partners.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
Including, but not limited to the following :
Clinical Oversight & Care Coordination
1. Provides day-to-day supervision of Home Health RNs, LPNs, and Home Health Aides.
2. Facilitates weekly Home Health Care Conferences and ensures care planning aligns with regulations and agency policy.
3. Reviews all OASIS assessments (SOC / ROC / Recert / DC) for accuracy, timeliness, and clinical completeness.
4. Conducts admission visits or other clinical visits as needed based on staffing and acuity.
5. Coordinates nursing care with the Therapy Clinical Manager to ensure interdisciplinary alignment.
6. Ensures timely documentation and addresses errors or coaching needs promptly.
Scheduling & Operations
7. Oversees weekly visit scheduling for nursing and HHA staff; approves PTO requests and manages staffing to ensure adequate coverage.
8. Oversees the on-call schedule for nursing (Home Health–specific), in collaboration with the DON.
9. Approves timecards and mileage for home health field staff.
10. Ensures timely ordering of clinical supplies and equipment required for Home Health patients; approves supply & Interpath lab ordering within agency guidelines.
11. Supports Intake in screening referrals and determining Home Health appropriateness.
Compliance & Quality Assurance
12. Ensures continuous survey readiness and compliance with CMS Conditions of Participation and state regulations in conjunction with DON.
13. Participates in QAPI activities (Yes—continues involvement), including audits, documentation review, and performance improvement projects.
14. Monitors clinical productivity and caseload distribution to ensure efficiency and safety.
Staff Support, Education, & Supervision
15. Participates in interviewing, hiring, onboarding, and competency validation of HH nursing staff (LVN / RN / HHA).
16. Conducts supervisory and ride-along visits per agency policy.
17. Provides ongoing staff education, feedback, and performance evaluation.
18. Participates in progressive discipline with the DON when necessary.
Communication & Community Interface
19. Acts as liaison between Home Health and physicians, hospitals, SNFs, discharge planners, and other community providers.
20. Supports the DON and CEO with community outreach, presentations, and marketing as requested.
Leadership & Administrative
21. Participates as a member of the clinical management team in program planning, process improvement, budgeting input, and staffing decisions.
22. Provides limited on-call backup support as needed (Yes—but reduced, primarily when DON is unavailable).
23. Performs other responsibilities as assigned by the DON or CEO.
SUPERVISORY RESPONSIBILITIES :
REQUIRED KNOWLEDGE AND BASIC SKILLS :
Knowledge of-
Nursing principles, practices, procedures, and nursing process.
Familiarity with and commitment to the agency mission.
Medical terminology, drug actions and interventions and medical record keeping practices.
Thorough understanding of family and team dynamics and the ability to relate supportively to both patient and family.
Medicare and licensure regulations, standards, policies and procedures as related to Home Health and Hospice services.
Principles of supervision and personnel practices, along with good verbal and written communication skills.
Ability to-

Supervise the performance of professional and ancillary health care personnel.
Deal tactfully with and maintain an empathetic attitude toward the patient / family.
Adapt nursing care to the emotional needs and behavior of patients.
Coordinate patient services to provide optimal comprehensive care.
Read, write, and verbally communicate effectively in English.
Establish and maintain effective working relationships with the CEO, staff, physicians and representatives of other health care agencies.
EDUCATION AND / OR EXPERIENCE & QUALIFICATIONS :
Graduate from an approved school of nursing.
Minimum one-year acute care nursing experience; two years of recent home care experience; two years of progressively responsible experience in Home Health and / or Hospice, which includes at least two years of supervisory experience; or any satisfactory equivalent of experience and training which demonstrates an ability to perform the above-described duties.
Possession of current Oregon and California Registered Nurse License or ability to get one within 60 days of hire.
Current Driver’s License.
PHYSICAL DEMANDS / REQUIREMENTS :
Position requires professional and personal skills to cope with stress consequent to work that involves a high degree of mental, emotional and physical demands.
Regularly required to use hands to reach, write, utilize telephone and computer, at times for extensive periods; sitting and standing frequently required. Occasionally required to walk, stoop, kneel, crouch or climb stairs. Ability to work flexible hours, including the sharing of 24-hour call. Physical requirements may include patient lifting, repositioning, etc.
Ability to drive in a variety of adverse weather conditions, occasionally for lengthy distances or at night.

WORK CONDITIONS / ENVIRONMENT :
The noise level in the work environment is usually moderate.
Employee may encounter various environmental situations in individual patient homes, including odors, tobacco smoke or animals.
Tasks may involve exposure to blood and body fluids.
Primary work location will be out of the Gold Beach and Brookings office with travel required.
Working hours are generally 8 : 00 a.m. to 5 : 00 p.m. Monday through Friday.
Employee will occasionally be required to work weekends, holidays or other hours.
Required to be available on-call through answering service.
