Weekend Admission Nurse (RN) (Full Time & Part Time Available)

Part time Registered Nurse Case Manager

Anticipated Territory : Oakland Area

Schedule : Must be available Full Days Every Saturday and Sunday (4 SOCs / day)

Also willing to consider Full Time : Saturday and Sunday plus 3 week days.

What you'll do :

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including :

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients' conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
