Summary
The Advanced Practice Provider (APP) works independently, and in collaboration with the supervising physician to deliver excellent primary and chronic care management to a geriatric and palliative care population within the community primarily through a telehealth platform. The APP provides services within the APP scope of practice including the management of symptoms or disease processes related to acute and chronic conditions, assisting with communication and coordination of care, timely disease-specific education, as well as developing treatment plans that align with the patient's goals throughout the disease process. The APP would provide recommendations for and / or orders including pharmaceutical interventions / medications, laboratory, and diagnostic services as needed, and will work closely with the patient and caregivers to maintain close engagement for optimal management. Assessment of needs may include utilization of additional IDT resources including social work or chaplaincy as needed. Due to the variety of variables that may impact navigation of care for this population, the APP must be flexible, demonstrate initiative, organization, and excellent professional communication with ability to build rapport rapidly with patients and their caregivers for high patient and caregiver satisfaction.
