POSITION : Hospice Clinical Liaison - Full-Time (Monday - Friday, 8 : 30am - 5 : 00pm)
POSITION SUMMARY :
The Hospice Clinical Liaison serves as a key connector between Hospice of the North Coast and the community. This position blends clinical knowledge with marketing, outreach, and relationship development. The Liaison is responsible for driving admission growth, increasing HNC's visibility, and strengthening referral partnerships across hospitals, skilled nursing facilities, assisted living communities, physician groups, and community organizations.
Ideal candidates are licensed clinicians such as RN, LVN, LCSW, or MSW who bring strong communication skills, clinical understanding, and a natural ability to build trust. This role requires initiative, professionalism, and a passion for ensuring that patients and families receive timely access to quality hospice care.
