Job Description & Requirements Specialty : Case Management Discipline : RN Start Date : 11 / 17 / 2025 Duration : 13 weeks 40 hours per week Shift : 8 hours, days, evenings Employment Type : Local Contract Job Title : Case Manager Registered Nurse (RN) Location : Tallahassee, FL Shift : Day or Evening | 8 : 00 AM – 4 : 30 PM or 12 : 00 PM – 8 : 30 PM | 1 weekend per 4-week schedule Contract Type : Permanent Hours per Week : 40 Key Responsibilities : Perform comprehensive assessments of patients’ medical and psychosocial needs. Develop, document, and communicate individualized case management plans. Lead interdisciplinary care coordination meetings to optimize patient care and outcomes. Conduct medical necessity reviews using approved criteria at defined intervals. Perform utilization reviews and communicate with third-party payors. Collaborate effectively with physicians, patients, families, and other healthcare providers. Identify potential barriers to care and facilitate appropriate discharge planning. Promote cost-effective care without compromising quality or safety. Advocate for patients with high-risk conditions, ensuring access to necessary resources. Participate in quality improvement and performance initiatives within the department. Maintain regulatory compliance and documentation accuracy. Qualifications : Required : Active Registered Nurse (RN) License in Florida or Compact License (must obtain FL license within 90 days if compact). Minimum of 3 years of recent acute care Case Management experience (within the past year). Candidates with 3 years of experience in Med / Surg, Telemetry, Neuro, ICU, PCU, or ED will also be considered. Associate's Degree in Nursing (ADN) or Diploma in Nursing. Preferred : Bachelor’s Degree in Nursing (BSN). Certification in Case Management, Nursing, or Utilization Review. Experience in Home Health or Insurance Case Management (must include 3 years of total acute care experience and at least 1 year within the last 5 years).
