Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

Deliver skilled, patient-centered nursing care in home health settings as a registered nurse providing comprehensive assessments and tailored care plans for patients in their residences. This full-time, field-based contract role offers the opportunity to make a direct impact on patient well-being through clinical expertise and compassionate communication.

Key Responsibilities :

• Conduct thorough OASIS assessments and develop individualized care plans
• Administer skilled nursing care, including wound care, medication management, IV therapy, and chronic disease monitoring
• Monitor and evaluate patient progress, effectively communicating changes with care teams and physicians
• Provide education and support to patients and families on treatments, medications, and preventive health
• Maintain accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team members such as therapists and social workers to ensure coordinated care
• Participate in case conferences, staff meetings, and ongoing professional development
• Comply with regulatory and agency standards for quality documentation and care delivery

Contract Details : This 13-week full-time contract offers mige reimbursement for travel within your assigned territory.
