Job Summary & Deliverables
The Residential Case Manager is a certified position that provides targeted case management and psychosocial rehabilitative services as indicated by the care plan to support clients in residential settings to meet their individualized goals. Certified Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.
Provides targeted case management services as described by the Medicaid manual within residential programsSupports clients in following individualized care plans and identifying case management goalsCompletes documentation on services provided within Valley’s standardsDistributes medications as necessaryObserves and collects client urine samples as necessaryTransports clients to appointments as necessaryRequirements :
