Title : Care Coordinator - Suffolk & Nassau Counties

Employment Type : Full-Time, Temp-to-Perm

Pay : $50,000 annually (DOE)

Schedule : Monday-Friday, 9 AM - 5 PM

Location : Hybrid Field Role (Copiague office 2 days, field visits in Suffolk / Nassau; Fridays may be remote)

About the Role :

We are seeking a compassionate and motivated Care Coordinator to join a growing healthcare team serving Suffolk and Nassau counties. This role is field-based and will require travel throughout Suffolk and Nassau with some time spent in the Copiague office.

Care Coordinators are the critical link between clients and the care they need, ensuring individuals with complex needs-particularly those with Serious Mental Illness (SMI)-receive access to the services and resources necessary to live stable, healthy lives.

Responsibilities :

• Manage a caseload of 40-50 clients, providing at least 8 billable contacts daily (mix of in-person, telephonic, and home visits).
• Conduct client assessments and develop individualized care plans.
• Provide transitional care support for clients moving from hospitals or facilities into the community.
• Connect clients to benefits and community resources (housing, SNAP, SSI / SSD, HEAP, transportation).
• Document all encounters and maintain compliance with agency and payer requirements.
• Travel throughout Suffolk and Nassau counties to meet clients where they are.
