The Field Care Manager - Behavioral Health (Social Worker) is responsible for assessing, evaluating, and managing members' physical, psychosocial, and environmental needs to help them achieve and maintain optimal wellness. This role focuses on developing individualized care plans, coordinating care, and connecting members with appropriate community and healthcare resources.
The position is home-based with 15-20% travel to conduct in-home visits with Medicare and Medicaid members within assigned areas of Wayne or Macomb County, MI.
Key Responsibilities
- Manage a defined caseload and complete assessments with members in their homes, community settings, or telephonically.
- Provide clinical guidance and collaborate with interdisciplinary care team (ICT) members to support individuals with medical and behavioral health complexity.
- Develop, implement, and modify Individualized Care Plans (ICPs) to ensure members receive appropriate services in the least restrictive setting.
- Identify and address barriers to care, advocate for optimal member outcomes, and coordinate access to medical, behavioral, and social services.
- Serve as the primary point of contact for the member, caregivers, and care team participants.
- Review and complete medical attestations and clinical documentation to ensure compliance and accuracy.
- Support members in accessing social, housing, educational, and community resources as appropriate.
Required Qualifications
- Active Michigan Social Work License (Bachelor's level minimum; Master's preferred).
- At least 2 years of post-degree clinical experience in a behavioral health setting.
- Experience working with adult populations and individuals with complex health needs.
- Knowledge of community health, social service agencies, and available local resources.
- Strong interpersonal and communication skills with the ability to establish rapport quickly.
- Proficiency with Microsoft Word, Excel, Outlook , and electronic medical record systems.
- Ability to work independently in a remote environment and conduct home visits within assigned regions.
- Must have a valid driver's license , auto insurance , and access to a reliable vehicle .
- Must maintain a private, secure home office space to ensure confidentiality during member interactions.
