Job Title : Case Manager - RN

• Acute hospital experience required.
• Epic experience required.
• No first-time travelers.
• No locals; 50-mile radius restriction.

Description :

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs.

The overall goal of the position is to enhance the quality of patient management and satisfaction.

This role promotes continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning.

The Case Manager must be a highly organized professional with great attention to detail, adaptable to frequent changes, and compliant with regulatory and departmental guidelines and policies.

Willingness to work weekends is required.

The daily patient census on weekends is 70, and 20 during the week.

Candidates must have two years of healthcare experience as a Registered Nurse.

Experience in care coordination, concurrent review, discharge planning, and appeals and denials is essential.

The selected candidate will collaborate with interdisciplinary teams to ensure comprehensive patient care.

Candidates should be skilled in utilizing InterQual criteria and Milliman guidelines.

The role requires comprehensive knowledge in admission criteria, medical necessity, and needs assessments.

The ability to manage complex patient cases effectively is crucial for success in this position.
