Medical Social Worker (MSW) - Office Based

Position Overview :

The Medical Social Worker - Office Based provides psychosocial assessment, counseling, and care coordination services to patients and families in an office-based healthcare setting. This role focuses on addressing emotional, social, and practical barriers to health and wellness.

Key Responsibilities :

• Conduct comprehensive psychosocial assessments to identify patient needs, strengths, and resources.
• Provide individual, family, and group counseling to address mental health concerns, chronic illness adjustment, and care transitions.
• Develop and implement treatment plans in collaboration with patients, families, and interdisciplinary care teams.
• Connect patients with community resources including housing assistance, financial aid, support groups, and home care services.
• Serve as patient advocate to ensure access to appropriate healthcare services and address barriers to care.
• Provide crisis intervention and safety planning as needed.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive patient care.
• Maintain accurate and timely documentation in electronic medical records.
• Participate in care conferences and interdisciplinary team meetings.
• Provide education to patients and families regarding disease management, advance directives, and healthcare navigation.
