Position Overview :
The Medical Social Worker - Office Based provides psychosocial assessment, counseling, and care coordination services to patients and families in an office-based healthcare setting. This role focuses on addressing emotional, social, and practical barriers to health and wellness.
Key Responsibilities :
- Conduct comprehensive psychosocial assessments to identify patient needs, strengths, and resources.
- Provide individual, family, and group counseling to address mental health concerns, chronic illness adjustment, and care transitions.
- Develop and implement treatment plans in collaboration with patients, families, and interdisciplinary care teams.
- Connect patients with community resources including housing assistance, financial aid, support groups, and home care services.
- Serve as patient advocate to ensure access to appropriate healthcare services and address barriers to care.
- Provide crisis intervention and safety planning as needed.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive patient care.
- Maintain accurate and timely documentation in electronic medical records.
- Participate in care conferences and interdisciplinary team meetings.
- Provide education to patients and families regarding disease management, advance directives, and healthcare navigation.
Qualifications :
Master's degree in Social Work (MSW) from an accredited program required.State licensure (LMSW, LCSW, or equivalent) required.Minimum 12 years of experience in healthcare or medical social work preferred.Knowledge of community resources, insurance systems, and healthcare regulations.Strong assessment, counseling, and care coordination skills.Excellent communication and interpersonal abilities.Proficiency with electronic medical records systems.Ability to work collaboratively within an interdisciplinary team.IMPORTANT :
