Family Health Centers (FHC) is seeking a dedicated and compassionate Care Coordinator to join our Population Health team, focusing on patients with Passport Molina insurance. This role is vital in supporting our mission to provide high-quality care by connecting patients with preventive services and closing care gaps.Key Responsibilities : Schedule patient appointments and follow-upsMonitor and complete preventive health measuresAlert providers to diagnoses and care gaps for risk adjustmentUse registries to inform care teams of needed preventive servicesCounsel patients on recommended health screeningsEnsure accurate data entry into electronic health records (EHR)Identify and support patients with language or communication barriersScan and upload documents into EHROutreach to patients over-utilizing emergency services and connect them with primary careIdeal Candidate : Organized and able to prioritize tasks independentlyStrong communication and interpersonal skillsPassionate about improving patient outcomesExperience in healthcare or care coordination preferredJoin a team that works hard because our patients deserve the best.
