Intake Coordinator

PurposeCare and its affiliate companies are looking for a qualified, dependable, honest, reliable, Intake Coordinator to support those caring for the needs of our clients. We have an immediate opening and are searching for someone who can provide high-quality intake support for our clinical teams.

PurposeCare is a leading provider of home care and home health services in the Midwest and provides home-based care services to more than 3,000 clients a month carefully coordinated to keep clients healthy and safe at home.     • Qualifications and Requirements :

  • Obtain patient information as needed
  • Evaluate and assess documentation to ensure accuracy and compliance
  • Ability to effectively communicate with office and clinical teams
  • Home health or medical intake experience preferred
  • Ability to successfully complete a background check and professional reference check
