Responsibilities : As a valued member of our team, you will :
- Collaborate closely with a diverse group of healthcare professionals to design and implement individualized home care plans.
- Perform comprehensive patient assessments, laying the foundation for uniquely tailored care strategies.
- Administer medications, offer wound care, and provide expert nursing interventions.
- Maintain a vigilant eye on patient progress, facilitating seamless communication within interdisciplinary teams.
- Employ OASIS documentation and the HCHB system to uphold meticulous and punctual record-keeping.

- Duration : 13-week contract position within the realm of home health care excellence.
- Schedule : Enjoy a full-time commitment with a 40-hour workweek (8 : 00 AM to 5 : 00 PM).
