A company is looking for a Nurse Navigator 1 - Site Disease for a full-time remote position.

Key Responsibilities

Triages new patient appointments and ensures timely scheduling

Educates patients on treatment plans and supports them throughout the care continuum

Collaborates with a multidisciplinary team to coordinate patient care and referrals

Required Qualifications

Bachelor's Degree in Nursing

Registered Nurse Licensing (RN)

Minimum of 1 year of oncology experience

Ability to assess patient needs and barriers to care

Experience in coordinating care with multidisciplinary teams
