Care Connector

A company is looking for a Care Connector.

Key Responsibilities

Provide program interventions based on established health management guidelines and member stratification

Support care management department interventions and conduct inbound and outreach calls to collect data

Assist members in accessing care through health plan benefits and community resources while maintaining accurate documentation

Required Qualifications, Training, and Education

High School Diploma or GED required; Associate degree preferred

Minimum of 2 years of work experience in a healthcare environment required

Minimum of 1 year of telephonic customer service experience required, preferably in healthcare

Medical terminology experience preferred

Training or certification as a medical assistant, home health aide, or similar healthcare paraprofessional preferred
