directing psychopharmacologic and medical services. Qualifications : The psychiatric nurse practitioner must be a licensed nurse practitioner with experience with the population served.

- The primary purpose of this position is to provide clinical services to all clients receiving ACT team services, which includes medication prescription, administration, monitoring, and documentation;
- directing psychopharmacologic and medical services; service coordination, including working with the ACT team to monitor each individual's clinical status and response to treatment;
- crisis assessment and intervention; symptom assessment and management; individual counseling and psychotherapy; substance use treatment;
- work-related services; activities of daily living services; social, interpersonal relationship, and leisure-time activity services;
support services or direct assistance to ensure that individuals obtain the basic necessities of daily life; and education, support, and consultation to individuals' families and other major supports.
We are seeking a part-time Nurse Practitioner to provide the following services :
- Supervise all medical / clinical aspects of the services delivered.
- Identify appropriate evidence-based or emerging best practices therapies to be employed by staff in delivering services.
- Consult with primary healthcare providers / other providers on the clinical aspects of the clients being served and the implications for those clients' physical healthcare needs.
- Coordinate with medical staff of other providers, e.g. state and community inpatient facilities.
Supervise all other medical staff on the ACT team.
- Assist in identifying, planning, scheduling, and implementing services for the client.
- Crisis phone / on-call duties are rotated amongst team members.
- Assist in the development of transition plans when clients move in and out of services.
- Provide written documentation as required.
- This is considered Light Work; no lifting over 20 pounds.
- Must be free from all illicit substance use.
- Demonstrate fidelity to ACT Model as measured by TMACT.
- At the discretion of team lead, supervisor, and / or management, this position may require additional duties not specifically listed above.
