Requirements :

2+ years RN Case Management in Acute Care

Minimum 6 months experience in discharge planning in a Level 1 trauma or large academic hospital

EPIC EMR experience required

BSN preferred; ADN accepted

Flu vaccine required (only medical exemption allowed)

Fingerprinting required (remote : allow 10 days)

Role Overview :

Primary Function : Discharge planning only (not UR)

Patient Load : 15–20 cases / day

Coverage Area : All inpatient units throughout the hospital

Population : Medical and complex patients; UMMC is the only Level 1 trauma center in the state

Collaboration : Work with Social Work team (15 full-time, 17 part-time staff); report to Social Work Manager
