Patient Enrollment Specialist is responsible for initiating, processing, and completing the onboarding and enrollment process of our existing and new patients into the An RPM (Remote Patient Monitoring) & PCM (Principal Care Management) Programs. This role ensures all required documentation is collected, insurance eligibility is verified, and patients are guided through the enrollment process in a professional and timely manner.
The specialist acts as a liaison between the practice, referring providers, and the patients to ensure a smooth transition and positive experience.
They work closely with patients to ensure the successful use of RPM devices (CGM and BGM), interpret the data collected, and provide guidance on patient health management.
This role requires a blend of technical, medical, and communication skills to optimize patient care through remote technologies.
Key Responsibilities Patient Onboarding :
Foster strong relationships with patients, helping them feel comfortable and confident in using RPM devices. Device Management :
Set up, configure, and manage RPM devices such as wearable health monitors, blood pressure cuffs, glucometers, and other monitoring equipment. Training & Troubleshooting :
Coordinate repairs or replacements when necessary. Data Review & Tracking :
Track and report enrollment metrics as requested by management. Patient Support :
Provide ongoing assistance, answer questions, and encourage patients to stick with the program. Record Keeping :
Maintain confidentiality of all patient information in accordance with HIPAA and practice policy. Collaboration :
