About The Position

Seeking a skilled Home Health RN to provide in-home patient care, perform assessments, manage wound care and medications, and coordinate care plans. Must work independently, ensure accurate documentation, and deliver compassionate, patient-focused care.

Requirements :

1–2 years of Home Health experience (OASIS preferred)

Active RN license + BLS

Strong assessment, communication, and time-management skills

Valid driver’s license and reliable transportation
