Compensation :

Base Salary : $215,000-$300,000(commensurate with experience and location)

Incentive Bonus : Quality-based performance bonuses

Position Overview

We are seeking a dedicated Primary Care Physician to join our growing team in Longiview, TX . Our innovative network of neighborhood primary care centers is designed to provide equitable, patient-centered, and value-based healthcare to local Medicare populations.

Our physicians are supported by multidisciplinary care teams and advanced clinical technology, allowing you to focus on what matters most- delivering quality outcomes and preventive care , not patient volume.

Key Responsibilities

• Deliver comprehensive, patient-focused primary care services within a value-based care model .
• Manage a patient panel of approximately 13-16 patients per day .
• Conduct visits averaging 20 minutes for regular appointments and 40 minutes for new or complex cases.
• Collaborate with a dedicated care team including a Medical Assistant, Scribe, and Social Worker .
• Utilize on-site lab services with phlebotomy support and access to RubiconMD for virtual specialty consultations.
• Maintain accurate documentation using Canopy and Greenway EMR systems .
• Participate in community health initiatives such as wellness classes, exercise groups, and social engagement activities.
