Join our dynamic and growing cardiovascular team, where you'll play a pivotal role in providing exceptional care with a focus on women's heart health and prevention. We are seeking a highly motivated and compassionate Cardiovascular - Heart Failure Advanced Practitioner to join our team in Phoenix, Arizona . You will play a crucial role in providing comprehensive care to patients with cardiovascular conditions, including but not limited to the following.

• Provide comprehensive cardiovascular care.
• Perform patient evaluations, diagnostic assessments, and manage follow-up care, ensuring continuity and high-quality outcomes.
• Collaborate with your supervising physician and a multidisciplinary team to develop and implement personalized treatment plans.
• Participate in community outreach, patient education, and cardiovascular research.
• Order, interpret and analyze diagnostic tests.
• Prescribe and administer medications and physical therapy as necessary.
• Participants in quality assurance and quality improvement for patient care.
• Works within a multidisciplinary team.
• Additional duties as assigned by leadership.

Dignity Health Medical Group (DHMG) employs approximately 350 providers and 850 support staff who cover a wide variety of specialties. Pay Range

$59.41 - $74.91 / hour
