Job descriptionActive License in The State of Indiana DEA Certification Board Certification or Board Eligibility in Family Practice or Internal Medicine Flexible Schedule. Monday Friday : Mon and Tue : 7 : 30a 6p, Wed and Th 7 : 30a-7p, Fri 7 : 45a-5p 4 or 5 Days a Week - At Least 36 Hours 20 Patients a Day Call by Phone Every 2 Months Seeing Patients of All Ages No Surgery, Standard Procedures

Their team of physicians, nurse practitioners, behavioral health providers, and staff deliver friendly, expert care to individuals and families. The client is offering a salary range and the following benefits :
- $180,000-$220,000 Based on Experience
- Productivity and Quality Incentive
- Student Loan Repayment Available through PSLF and NHSC
- 144 Hours of PTO + 9 Paid Holidays
- 4 days of CME with $ 1500 Reimbursement
- 40 1K Retirement Plan - 4% Match After One Year
