About Haven Home Health

As an Occupational Therapist (OT) with Haven, you will act as a partner to your patients throughout their plan of care. In the Gainesville, FL area, you will help restore motion, optimize function, and ensure safety in the home. Your work will focus on improving quality of life and movement potential through promotion, prevention, diagnosis, treatment, intervention, and rehabilitation. Working under a physician-established Plan of Treatment and in collaboration with patients and caregivers, you will help guide a healthy recovery and support a better way of living.

Key Responsibilities :

  • Understand and adhere to established policies and procedures
  • Improve or minimize residual physical disabilities of patients
  • Help patients return to their optimum and productive level within their capabilities
  • Participate in patient care planning with all healthcare personnel
  • Take initial histories and perform evaluations
  • Deliver all skilled procedures as ordered by physicians
  • Consult with physicians regarding changes in treatment
  • Educate patients and their families on home programs and activities of daily living
  • Attend patient care conferences as scheduled
  • Prepare clinical and progress notes
  • Assist in developing and revising the plan of care as needed
  • Consult with families and agency personnel
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within agency-defined time frames

    • Haven Qualification Standards :

  • Current Occupational Therapy license in the state of Florida
  • Acceptance of the philosophy and goals of Haven Home Health
  • Ability to exercise initiative and independent judgment
  • Intensive education and clinical expertise
  • Ability to manage patients with diverse personalities
  • Current knowledge of treatment practices
  • Drive to continually learn and grow professionally
  • Professional behavior and excellent interpersonal communication skills
