Clinical Care Registered Nurse

Certifications : CPR certification by AHA or willingness to obtain

Must-Have :

• Bachelor's degree preferred in Science of Nursing; Associate's degree required
• Minimum of one year experience working with the frail, elderly, or long-term care population
• Minimum of one year home care experience preferred
• Minimum of one year wound care experience preferred
• Reliable transportation and valid driver's license
• Fully vaccinated or willing to be fully vaccinated against communicable diseases

Description :

The Clinical Care Registered Nurse (CCRN) role provides direct nursing care to participants.

This role works independently in the application of advanced nursing knowledge and skills.

The CCRN is able to manage complex clinical situations.

This role collaborates with both licensed and non-licensed personnel.

Key responsibilities include triaging emergent situations and applying advanced nursing knowledge independently.

Monitoring acute and chronic medical conditions in coordination with Primary Care Providers is essential.

The role utilizes care plans independently to provide both clinic and home-based direct care.

Administering treatments, procedures, and medications as prescribed to participants is required.

Regular home visits are necessary to provide care and treatment as ordered by the Primary Care Provider.

These visits may include wound care, lab draws, nurse assessments, and other nursing tasks.

The work environment includes a visible presence while working in various locations and is subject to frequent interruptions.

Communication with staff, participants, caregivers, and other organizations is necessary.

Flexibility is required to work beyond normal working hours, including weekends and holidays.
