Job description
A company is looking for a Nurse Case Manager - NICU who must reside and be licensed in South Carolina.
Key Responsibilities
Assess the needs of members and develop individualized care plans
Monitor and coordinate care plans, ensuring effective provision of services
Provide education and referrals to community resources for members
Required Qualifications
Current, unrestricted Registered Nurse (RN) license in South Carolina
Preferred college degree
Three or more years of clinical experience in a Level III or above NICU
CCM certification required within specified timeframe
Experience with data entry into a database or EHR
