We are currently seeking an experienced :

HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER

Per Diem

(Covering Onslow and surrounding counties)

Job Description :

• Implements palliative care program requirements
• Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated
• Conduct face to face encounter to provide re-certification information to Medical Director / Associate Medical Director for Hospice Patients
• Provide clinical findings of that face to face encounter Primary care physician, and / or supervising Medical Director / Associate Medical Director
• Communicate with patients / families at all levels.
• Communicate with all agency personnel
