We have an opportunity for a Counselor to join us and make Lake Wellness Center your new "work home". This position is Monday to Friday, 9am-5pm.

If you join our team, you'll enjoy the following responsibilities :

• Provide clinical services via individual, group and family therapy.
• Provide crisis intervention and referrals for patients.
• Participate in clinical assessments and screenings to aide in the admissions process.
• Write and submit clinical master admission, discharge and updated treatment plans, and clinical updates for UR review.
• Conduct assessments and evaluations based on the model / treatment criteria.
• Engage patients at admission and through their treatment episode to ensure delivery of the best patient care with unconditional positive regard.
• Document patient notes, interventions, phone calls, and overall patient data, including symptom monitoring.
• Document weekly progress notes and weekly progress questionnaires.
• Keep all patient charts up to date.
• Work collaboratively with the medical and clinical team and provide immediate notification of unusual or potentially high-risk findings.
• Actively participate in weekly medical and / or clinical staffing of patients.
• Contact current patients on a regular basis to keep patients engaged.
• Provide support for clinical services to other team members when needed.
• Adhere to rules and regulations regarding treatment, such as group size, caseload and referrals.
• Aggressively implement continuous quality improvement initiatives.
• Other duties as assigned.
