Wound Care Nurse Practitioner (Part-time)- Merced - CA

Shift : Days

Multiple locations : Modesto / Merced / Madera / Sonora / San Andreas - CA

Job Summary & Responsibilities :

Evaluate and treat both acute and chronic wounds

Create and implement individualized care plans

Monitor patient progress and modify treatment plans as necessary

Document wound status and care provided for all new admissions

Education, Licensure, and Experience :

Active California RN license and Licensed Nurse Practitioner (NP)

Minimum of 2 years of wound care experience , with strong knowledge of treatment protocols and industry best practices

Proficient in Electronic Medical Records (EMR) systems

Skilled in skilled nursing facility (SNF) settings ; Wound Care Certification preferred
