As an Assisted Living Coordinator at River Bend Health and Rehabilitation, you will play a vital role in ensuring the highest level of care and support for our assisted living residents. This is a dynamic and challenging opportunity to join a dedicated team of professionals who are passionate about making a difference in the lives of seniors.

Responsibilities :

• Coordinate Care : Develop and implement personalized care plans for each assisted living resident, in collaboration with the interdisciplinary team
• Medication Management : Manage medication administration, record keeping, and documentation for all assisted living residents
• Resident Services : Assist residents with daily living activities, such as bathing, dressing, and grooming, as needed
• Communication : Maintain effective communication with residents, families, and caregivers to ensure seamless transitions and continuous care
• Observation and Assessment : Monitor and assess resident changes, reporting concerns to the interdisciplinary team and implementing interventions as necessary
