Locums Nurse Practitioner - Mishawaka, IN Assignment Dates - 6-month assignment to start Coverage Need - Full-time preferred (Part-time considered) - Scheduled clinic hours with on-call responsibilities : - On-call one week on, one week off - Weekday on-call : 4 : 30 PM to 8 : 00 AM - Weekend on-call : 24-hour coverage - Phone-only on-call (no travel required) Position Details - Location : Mishawaka, IN - EMR : TruChart (transitioning to Epic in March) - Geriatric experience required - Provides care within a PACE (Program of All-Inclusive Care for the Elderly) model : - Assessments, care planning, treatment, participant education, and team collaboration - Oversight of the clinic and contracted nursing home rounds - Interdisciplinary Team (IDT) participation - Documentation of participant care and medical condition updates Submissions Requirements - Active IN license- Clean History- Board Certified (ANCC or AANP)- Up-to-date CV with explanations for gaps >
