RN Case Manager

Role Overview :

We are seeking a Motivated RN to join our team in Grand Junction as a Case Manager to assist with Private Duty Nursing Cases, Skilled Nursing Visits and the exciting new Market Growth and Development we are looking to make in the Western Slope.

$2,500 Sign-On Bonus!

Pay :

$83,000–$87,000 annually, with flexibility based on experience and overall fit

Requirements

Key Responsibilities :

Provide private duty nursing care to patients in their homes to help with caseload growth until full to continue in a supervisory capacity.

Provide skilled nursing visits to establish census.

Administer medication, treatments, and therapies as prescribed by the physician.

Monitor patient's condition and report any changes to the healthcare team.

Assist with activities of daily living and provide emotional support to patients and their families.

Collaborate with other healthcare professionals to ensure optimal patient care.

Maintain accurate and complete medical records using web-based electronic medical records system.

Assist with marketing and market growth development in Grand Junction and the surrounding areas.

Partner with talent acquisition and patient alignment.

Attend careers fairs, partner with local universities and attend various marketing opportunities as needed.

Qualifications :

Active RN License.

Previous Home Health experience preferred.

Current BLS Certification.

Valid Driver's License and Vehicle Insurance.

Strong time management and prioritization skills.

Excellent communication, assessment skills and attention to detail.
