Permanent Geriatric Medicine Medical Director – Pensacola, FloridaSetting : Multidisciplinary Care Model (includes community-based settings)Visa Sponsorship : Not offered Position Overview :

• Conduct H&Ps on new participants and perform event-based or periodic reassessments
• Develop medical treatment plans and provide medically necessary services across participant care settings
• Participate in intake and assessment evaluations with applicants and caregivers
• Discuss prognosis and care plans with participants and families to support shared decision-making
• Collaborate across nursing and interdisciplinary teams on treatment planning
• Actively contribute to team / family conferences and medical decision-making
• Educate patients and families on chronic conditions and health maintenance
• Supervise care plans developed by the interdisciplinary team
• Order and evaluate labs / diagnostics, document results systematically
• On-call responsibilities as required
